El Pasado Perfecto Continuo se usa para hablar sobre cosas que habían estado ocurriendo durante un período de tiempo antes de otro evento en el pasado. Es decir, se utiliza para decir lo que había estado pasando o lo que alguien había estado haciendo.
Estructura
(+) Sujeto + had + been + verbo en gerundio (-ing)
(-) Sujeto + hadn't + been + verbo en gerundio (-ing)
(?) (_) + had + sujeto + been + verbo en gerundio (-ing)
(_) = palabra interrogativa
Es opcional
Se utiliza en preguntas abiertas, que no pueden ser respondidas con "Sí/No"

Had es el pasado simple del verbo 'to have' (haber/tener), en este caso utilizado como verbo 'haber' en su forma personal (de haber hecho) porque alguien había estado realizado una acción.
Sigue siendo un Past perfect a nivel estructura. Donde el verbo 'haber' es lo único que esta en pasado. Pero, en este caso, el verbo en pasado participio debe ser 'been', participio del verbo to BE (estar); y se agrega el verbo principal en gerundio (-ing, nuestro clasico –ando/-endo (jugando, comiendo) porque algo había estado pasando.
Ejemplos:
Water had been boiling for five minutes when I turned off the stove. (El agua había estado hirviendo por 5 minutos cuando apagué la hornalla)
I had been waiting for two hours by the time the bus arrived. (Yo había estado esperando por dos horas para cuando llegó el colectivo)
I hadn't been waiting for hours by the time the bus arrived. (Yo no había estado esperando por horas para cuando llegó el colectivo)
How long had you been waiting by the time the bus arrived? → I had been waiting for two hours. (¿Cuánto tiempo habías estado esperando para cuando llegó el colectivo? → Había estado esperando por dos horas)Tom and Jerry were tired because they had been playing football all day. (Tom y Jerry estaban cansados porque habían estado jugando al fútbol todo el día)
Tom and Jerry hadn't been playing football all day. (Tom y Jerry no habían estado jugando al fútbol todo el día)
Why were Tom and Jerry tired? → Because they had been playing football all day. (¿Por qué estaban cansados Tom y Jerry? → Porque habían estado jugando al fútbol todo el día)
Had Tom and Jerry been playing football all day? → Yes, they had. / No, they hadn't. (¿Habían estado jugando al fútbol todo el día? → Sí / No)
